Background

Grand County Public Health (GCPH) is soliciting proposals from qualified individuals or organizations to conduct a community health assessment and health issue prioritization for the Grand Improvement Process (PHIP). The PHIP is the strategic response to the Colorado Public Health Improvement Act. This Act requires that each local health department in the state: a) Prepares a health improvement plan; b) Bases this plan on a community health and system capacity assessment; c) Sets priorities for providing services; d) Provides or arranges for provision of equitable, quality, core public health services; and e) Ensures that these processes are collaborative, consistent with state plans, and aligns with resources. These public health responsibilities extend indefinitely into the future and are built around recurring five-year cycles of planning, implementation, and evaluation.

The awarded contractor will facilitate and coordinate activities associated with this project and complete the entire project by October 15th, 2023. The work will be in partnership with GCPH & the project management team and their stakeholders. The work entails providing a variety of facilitation duties related to the community health assessment and prioritization of issues. Key areas of interest include local health status and health system capacity, prioritization of health issues, administration and collection of assessments and data, and recommendations for public health improvement planning. Work will aim to engage and access experiences of a diverse group, including public health and community service experts. The project will also utilize secondary data, research other data collection in the county.

Budget Range: $40,000 to $65,000

Selection Process

Based on responses to this request for proposal, top candidates may be asked to demonstrate their knowledge and experience via interview. Based on responses and interviews, the Grand County Public Health Advisory Committee (GCPH-AC) will decide on the top candidate for contract award and negotiations.

Qualified candidates will have 5 or more years experience in community facilitation, with a minimum of 2 years in community-based strategic planning (must include assessment, prioritization, planning, and evaluation). Delivery, collection, and analysis of surveys experience is a must.

Knowledge, Skills, and Abilities:
  • Training, experience, and proven track record in facilitation.
  • Training, experience, and proven track record in community-based, participatory methods.
  • Strong computer literacy skills in word processing, spreadsheet, and presentation software (preferably Microsoft Word, Excel, PowerPoint, Access, Google Drive, Virtual Meeting applications and Outlook).
  • Strong written/oral communication, presentation, and analytical skills, with the ability to effectively communicate complex technical data and statistics to both professional and lay audiences.
  • Strong organizational skills and ability to prioritize workload effectively to meet deadlines.
  • Ability to plan and work effectively with agency staff, community partners, and the public.
  • Ability to work well independently, as well as with a team.

Special Requirements:
  • Must have access to reliable transportation, readily available for business use.
  • Must provide own computer equipment, office supplies, and work space.
  • Must provide proof of employer’s insurance including general liability, personal injury, professional liability, automobile liability and worker’s compensation coverage.

Preferred Capabilities:
  • Familiarity with the Public Health Act of 2008 and the Colorado PHIP initiative.
  • Ability to coordinate logistics for group meetings, interviews, and work.
  • Master’s degree in public health.
  • Facilitation work experience for Grand County.
  • Fluent Spanish language skills written and verbal

Scope of Work

PHIP entails the following seven phases of the Colorado Health Assessment and Planning Process (CHAPS):

  1. Organize for success.
  2. Identify and engage stakeholders
  3. Conduct a community health assessment
  4. Conduct a capacity assessment
  5. Facilitate a prioritization process with a focus on solutions
  6. Develop a local public health improvement plan.
  7. Report into the state system.
  8. Track progress and evaluate.

The contractor will focus on phases 3 (community health assessment), 4 (health system capacity assessment) and 5 (prioritization) above. Information from phases 1 and 2 will support the contractor’s work, while the contractor’s deliverables will provide the foundation for phase 6 (plan). Phases 7 (reporting) and 8 (evaluation) are ongoing.

The contractor will report to the Advisory Committee and/or the Grand County Public Health Director.

Description of Work

The contractor’s primary communication will be with the GCPH-AC. The contractor will solicit input from staff and community partners on health systems capacity assessment and prioritization of strategic health issues, and communicate this information clearly.

All work will be structured by, contribute to, and benefit from state CHAPS tools, templates, and technical support and the PHIP mandates.

Contractor Tasks

The major contractor tasks are detailed below. Deliverables for each include submission of presentations, documentation of task process (e.g. steps, participants, and tools necessary to replicate the process in the future), and publication-ready executive summary of outcomes. The GCPH and health assessment management team will review, revise, and approve deliverables.

Contractor Tasks:

  1. Coordinate & communicate with GCPH-AC and stakeholders.
  • Organize meetings with stakeholders, community organizations, nonprofits, governmental agencies and others in building collaborative partnerships.
  • Develop and make public presentations to leadership, staff, and stakeholders about the process and findings as directed by project managers
  • Reports on the status of project activities to project managers.
  1. Facilitate meetings
  • Facilitate project management meetings, steering committee, and stakeholder meetings
  • Work with project managers to develop agendas, identify meeting objectives and determine necessary materials and equipment.
  • Utilize facilitation tools to manage meetings.

  1. Conduct Community Health Assessment that includes data about health status and risk factors in each local community in coordination with public health and community stakeholders.
  2. Conduct Capacity Assessment which includes public health capacity assessment and capacity of performance of core public health functions in the county.
  3. Facilitate prioritization process in Grand County. The criteria and a proposed process are described in the CHAPS Phase V. Selecting areas of focus will help define the local public health improvement plan. Document changes in the capacity to provide core public health services.
  4. Production of Grand County health assessment report including prioritization of identified health issues, strategies and solutions. Report will include an executive summary.

Evaluation Criteria:

Submissions will be evaluated on the detailed work plan to complete the major tasks listed above, previous experience and reference. In addition, the following terms and conditions apply to all submissions.

  1. Grand County reserves the right to reject any or all bids, to waive irregularities or informalities, and to conduct such investigations as they deem necessary to select the most responsible and responsive person or entity that has submitted a proposal (“Proponent”) and select the proposal that best meets the requirements of the County and the public interest.

2. Grand County reserves the right to conduct such discussions or negotiations with Proponents or other entities as they deem necessary to assist in the evaluation of any proposal, to secure maximum clarification and completeness of any proposal, and select the proposal that best meets the requirements of the County and the public interest.

3. All proposals submitted must be valid for a minimum period of 90 days after the date of the proposal opening. Each Proponent must submit with the proposal a list of all subcontractors, independent contractors or subcontractors employed or proposed to be employed by the Proponent in the performance of the contract.

4. In addition to all other provisions of this request, the following criteria shall be considered in evaluating the proposals, which criteria are not ordered in any particular priority:

  1. Cost
  2. The reputation, experience and efficiency of the Proponent.
  3. The ability of the Proponent to perform the contract or provide the goods and services within the time specified.
  4. The comparative quality of the goods and services bid.
  5. The Proponent’s performance under previous contracts with Grand County, if applicable.
  6. The ability of the Proponent to provide future maintenance and services.
  7. The number and scope of conditions attached to the proposal.
  8. The Proponent’s interest in the project, as well as their understanding of the project scope and the specific requirements of the County.
  9. The Proponent’s schedule of completion.
  10. The application of all the above criteria to any sub-consultants, subcontractors or products to be utilized by the Proponent.

6. All proposal documents submitted shall become the property of Grand County.

7. Proponents must be prepared to demonstrate qualifications by submitted evidence to Grand County such as financial data, previous experience, authority to conduct business in the jurisdiction where the project is located, and other requirements as may be specified in the contract documents. The County makes no commitments to any Proponent until such time as the County approves the negotiated contract.
